    INSiGHT Scans Explained: What They Measure and What They Don't

    INSiGHT scans are a set of non-invasive measurements taken with a handheld sensor that rests against the skin. There are no needles, no radiation, and nothing enters the body. The scan itself takes a few minutes and most children find it unremarkable.

    They measure three things: skin temperature symmetry along either side of the spine, the electrical activity of the muscles supporting the spine, and heart rate variability. Taken together, these give us a consistent baseline we can re-measure later, so that changes over months are something we can show you rather than something you have to take on faith.

    What they are not is a diagnostic test. These scans do not detect disease, they do not evaluate organ function, and they cannot tell you whether your child has a medical condition. That distinction matters enough that most of this article is about it.

    What Each Scan Measures

    Thermal. A sensor reads skin temperature down both sides of the spine and compares left to right. Skin temperature is partly regulated by the autonomic nervous system, so persistent side-to-side differences are of interest to us. The reading is of skin temperature — nothing more direct than that.

    Surface EMG (sEMG). Sensors on the skin pick up the electrical activity produced by the muscles running alongside the spine. This gives a picture of how much work those muscles are doing and whether the workload is balanced side to side. It measures muscle activity, not nerve damage.

    Heart rate variability (HRV). This measures the small beat-to-beat variations in heart rhythm, which reflect autonomic nervous system activity. HRV is a well-studied measure in adults. In young children the normal ranges differ substantially from adult ranges and are more variable, so we interpret it cautiously, and for infants we generally don't rely on it at all.

    Are They Safe for Children?

    Yes, in the straightforward sense that nothing is emitted, injected, or absorbed. A sensor touches the skin. There is no radiation exposure, unlike an X-ray. The practical limit with children is not safety but cooperation: a scan requires sitting reasonably still for a minute or two, which rules out most infants and many toddlers. When a child can't sit for a scan, we skip it. It is not necessary in order to evaluate your child.

    The Honest Part: Reliability Versus Validity

    Here is a distinction worth understanding, because it separates what these scans genuinely offer from what they're sometimes claimed to offer.

    Reliability means an instrument gives the same answer twice. On this, the equipment does well. A study of 100 subjects using a commercially available infrared paraspinal scanner found excellent intra-examiner and inter-examiner reproducibility, with intraclass correlation coefficients between roughly 0.96 and 0.98. A separate reliability study of paraspinal thermal scanning reported similar figures and concluded that changes seen in properly performed scans are more likely to reflect real physiological change than equipment error. Static paraspinal sEMG has also shown high reproducibility.

    Validity is a different question: does the measurement mean what someone claims it means? Here the evidence is far weaker. The American Academy of Neurology's technology assessment on the clinical utility of surface EMG has been critical of using surface EMG as a diagnostic instrument, and surface EMG scanning marketed for detecting spinal problems has drawn sustained criticism from mainstream medical reviewers.

    Both things are true at once. The instrument reliably measures what it measures. Claims that go beyond that are not supported.

    What These Scans Cannot Do

    You may encounter chiropractic websites claiming that scans like these detect nerve interference, identify which organs are under stress, or catch problems before symptoms appear. We don't make those claims, and you should be cautious of anyone who does.

    Specifically, an INSiGHT scan cannot:

    • Diagnose any medical condition
    • Assess the health of internal organs
    • Detect nerve damage
    • Screen for illness
    • Determine whether your child needs chiropractic care, independent of history and physical examination
    • Substitute for evaluation by a pediatrician

    If your child has a symptom that concerns you, a scan is not the answer to it. Evaluation by your pediatrician or an appropriate specialist is. Chiropractic care does not diagnose or treat underlying medical conditions.

    So Why Do We Use Them?

    For one reason, mostly: tracking.

    Chiropractic care for children often addresses things that are hard to measure — tension, asymmetry, how a body is moving. Without some objective marker, assessing progress means asking a parent whether things seem better, which is a genuinely useful question but a subjective one. Scans give us a second, repeatable data point.

    They are also a communication tool. It is easier to show a parent a side-by-side comparison than to describe what our hands found. During a first visit we run the baseline and walk you through it in plain language on the day, then re-scan periodically.

    That's the whole of it. Useful for tracking change and explaining findings. Not a diagnostic test, and not the basis on which we'd ever recommend care by itself.

    Questions Parents Ask

    "Do I have to have my child scanned?" No. It is a tool we offer, not a requirement.

    "Will a bad scan mean my child needs a long care plan?" A scan result does not generate a recommendation by itself. History and physical examination do most of that work, and we re-evaluate rather than locking in an initial plan.

    "Can I have a copy of the report?" Yes. Ask for it.

    "How often will you re-scan?" Periodically, when enough time has passed for a comparison to mean something. Scanning frequently enough to show week-to-week movement mostly shows noise.
